Patient Records and how we use them

 

To Register or Change your Details

First check that you are now living in our practice area, then to register bring your completed "medical card" from your previous GP (you should have received this in the post when you registered with your last GP) to reception. If you cannot find this, there is a form you can collect from our front desk which you can complete. Details such as telephone numbers, date of birth and previous GP are especially important.

Changes of address/telephone number/name can be notified on a special form available from reception, or in writing to us. If you have moved outside our practice area, you will have to reregister elsewhere. If we find out you have moved outside our area and not told us, we will remove you from our list. This is because we are at the upper limit of the number of patients we can safely provide high quality care to, and with more houses being built in Battle every day we cannot keep on patients with other GPs nearer to them.

If you are a student home from University or Boarding School, or are going to be in our area for less than 3 months, you can register as a "temporary resident". Please make sure that the reception team know you are a temporary resident, so they can give you the right form.

If we cannot take you on as a patient, or we remove you from our list either because you move, or because of some other reason, you can be "allocated" to a GP by phoning 01273 485300.

To Get Test Results

If your GP has not already asked you to make an appointment to discuss the results of blood and other test results, you can phone up, in the afternoon 7 days later (3 working days for Urine and Stool samples, 10 working days for x-ray results) and speak to the reception team, who will tell you if the results are satisfactory, or whether your GO wishes to speak to you or see you, or give you a prescription.

What you need to know

Health and social care professionals caring for you keep records about your health and treatment to ensure that you receive the best possible care.  These may be written down (manual records) or held on a computer.

Your records are used to guide and administer the care you receive to ensure:

  • Your health and social care professionals, such as doctor, nurse and social worker, involved in your care have accurate and up-to-date information to assess what care you need.

  • Your information is always available should you need to be referred to a specialist

  • There is a good basis for assessing the type and quality of care you have received

Your information may also be used to:

  • Investigate complaints, legal claims or untoward incidents

  • Make sure our services meet your needs now and in the future

  • Enable good management of services

Everyone working with your information has a legal duty to keep it confidential.  Our guiding principle is that we are holding your records in strict confidence.  Anyone who receives information from us is also under a legal duty to keep it confidential.

The principal partner organisations with whom information may be shared are:

  • Hospital, Community and Mental Health Trusts, Primary Care Trusts (PCTs), General Practitioners (GPs), Ambulance Services and Social Services (if involved in your care).

  • Your information is subject to strict agreements describing how it will be used and, only if necessary for your care, may also be shared with:

  • Local Authorities, NHS Common Service Agencies, Voluntary Organisations and other care providers.

Your records are managed in accordance with the Data Protection Act 1998 and under this you have a right to access your records.
